title: The assessment of a change in cardiorespiratory reserve, functional capacity, risk factors of coronary heart disease and quality of life as the result of complex cardiac rehabilitation in patients on waiting list for surgical revascularization of the myocardium.

duration: 01.01.2003 - 31.12.2006
description: The previous investigations performed in the Department of Sports Medicine and Rehabilitation, University of Tartu, have convincingly demonstrated that complex cardiac rehabilitation, involving structurized exercise therapy, psychological counselling and risk factor modification in early period after surgical revascularization of the myocardium has extremely favourable influence on several functional, biochemical and psychological indices. Until now it is unclear whether it would be possible to gain the similar results in patients on waiting list for surgical revascularization of the myocardium. These particular subjects have extremely complicated cardiovascular condition due to insufficient perfusion of the myocardium as the result of severe coronarosclerosis, subsequent heart failure and significantly decreased functional capacity and health-related quality of life. The planned investigation has additional significance beacause of the waiting time for revascularization is prolonging from year-to-year . Actually, the patients on waiting lists have been left out of the focus of medical care and interest as their further treatment has already been planned. At the same time the mortality risk among these subjects is high, approximately 4%.

Due to above-listed circumstances it offers high scientific interest whether

* the indices of cardiorespiratory reserve and functional capacity could be favourable modified with regular exercise conditioning despite of severe ischaemic damage of the myocardium
* is it possible to modify risk factors of coronary heart disease sufficiently in the particular subgroup
* health-related quality of life can be favourably modified until surgical revascularization will be performed
* rehabilitation could improve the results of surgical revascularization
* complex rehabilitation programme is cost-effective, diminishing hospitalization due to cardiovascular disease
