description: | Cardiovascular diseases (CVD) are a major cause of mortality and morbidity in both females and males in many countries. In Estonia, the primary cause of death is CVD in 46% of males, and in 61% of females. It is widely suggested that in-hopsital mortality of females after AMI is higher when compared to men. As a possible source for gender related differences, the differences in introduction of evidence-based treatment as well management startegies have been proposed. Complications of AMI and mortality are closely associated with cardiac care delivery in hospitals. Optimal treatment is of primary importance to attain the best clinical outcome. In Estonia, there is lack of information regarding management and outcome of AMI in females and males. The aim of the present study is to investigate the differences of in-hospital management and 1-year outcome of acute myocardial infarction (AMI) between men and women. Development of a systematic evidence-based approach for management of all AMI-patients by coordinating national quality improvement efforts is expected as a result of the project. The Internet-based Estonian Acute Myocardial Infarction Registry (EMIR) incorporating data from different Estonian hospitals treating AMI-patients serves as an excellent predisposition for the collection of standardized database of real practice patterns. World practice suggests that national registries consisting of consecutive cases of AMI provide the most comprehensive and accurate database reflecting the real clinical activities. The present project has important strengths of being community-based which should minimize bias and ensure the generalizability of the results. The present project includes clinical details (78 parameters) from EMIR for consecutive AMI-patients from different hospitals in a time period of February 1 2004 - January 31, 2006. EMIR-database enables to adjust possible gender-related differences for a number of clinical factors, and consequently reveal relevant associations which determine gender differences after AMI, representative of the Estonian population. |
