description: | In Estonia, despite of the decrease of HBV incidence rate 1,8 times in 2002 in comparison with 2001, it makes up 31,6 % in the structure of morbidity with viral hepatitis. The share of chronic HBV makes up about 20 % out of all registered chronic hepatitis. Studies in molecular epidemiology have classified HBV on the basis of complete genome or S-gene sequences into 8 genotypes designated A-H, which have a characteristic distribution. Four major subtypes of HBV were identified according to the combination of the antigenic epitopes of HBsAg: adw, ayw, adr and ayr. Subtypes have different geographic distribution. It has been suggested that the viral genotype and serotype may correlate with difference in clinical course and outcome of the infection. Despite of increasing data regarding the characterization of these specific viral variants (HBV mutants) their clinical significance has still remained unclear. Aims of investigation are: to determine genotypes and serotypes of HBV isolated from patients with different forms of HBV infection, to make phylogenetic analysis of these strains; to characterize the HBV isolates according to genetic structure - for detection of mutant variants; to elucidate the correlation between genotype, serotype, mutants of HBV and the clinical forms of HBV disease. The results of the study will be of epidemiological significance about different genotypes and their genetic variants of the HBV in Estonia with possibly valuable information to predict the course of the disease. The results will be presented as a part of the dissertation. |
