description: | The present investigation focuses on the determination of the optimal mass of the thyroid remnant with the aim to ensure permanent postoperative euthyrosis. The mass of the thyroid remnant is the only factor that the surgeon can influence directly in the treatment of hyperthyrosis. In the case of a too large thyroid remnant the patient is endangered by recurrent thyrotoxicosis, while in the case of a too small thyroid remnant there develops hypothyrosis Main aims of research: 1) To determine the optimal portion of the thyroid remnant in grams in order to ensure permanent euthyrosis in the postoperative period. 2) To determine the optimal mass ratio of the thyroid gland to the thyroid remnant (expressed in percentage) for ensuring permanent euthyrosis in the postoperative period. 3) To establish the relationship between localisation (paratracheal versus apical) and postoperative disease course. In addition, it would be possible to obtain statistical data about postoperative complications as well as to apply them in the assessment of treatment results and in their comparison with the data of international practice. Working hypotheses: 1) The mass of the thyroid remnant ensuring permanent postoperative euthyrosis is 1-2g. 2) The mass ratio of the portion of the thyroid gland to be removed to the thyroid remnant appears important, i.e. in the case of a smaller thyroid gland the proportion of the remnant should be minimal, while in the case of a larger thyroid gland the mass of the thyroid remnant can be larger too. 3) The thyroid remnant left in the apical region of the gland has better blood supply, which guarantees also a better function of the thyroid remnant in the postoperative period. |
