description: Joint replacement is one of more actual problems in orthopaedic surgery. In the period 1990-2002 2316 primary hip replacement operations and 876 knee replacement operations were done at the Clinic of Traumatology and Orthopaedics of Tartu University. The rate of revision operations was 5.9 percentage in hip and 3.4 percentage in knee surgery. The main reason for hip revision operations is aseptic loosening. Heterotopiv ossification (HO) is also a frequent complication. The main purpose of the present investigation is to improve clinical outcome of hip and knee replacement using additional medicamentous and rehabilitation measures. Prevention of HO is investigated using nonsteroidal antiflammatory drugs and also their influence in connection of loosening of prosthetic components. Histological and molecular biological investigations of interface tissue are performed. Kinematic and dynamic gait analysis and strength characteristics of knee extensors are investigated pre and postoperatively with purpose to achieve better functional results.
